In order to aid us in the birthday and other programmes for underprivileged kids, please fill in the questions below.

1 Joining the birthday programme is an annual commitment. Are you willing/able to commit for at least one event a month for 12 months? If able to take up a shorter period indicate length.
2 Tell us why would you like to become involved in this birthday programme?
3 Do you have a car? Are you willing to use it for the monthly event?
4 Do you have any other ideas or capabilities that can help with the birthday programme and these kids? (e.g. Face painting, magician, puppet making etc). Please elaborate.
5 Is there anything preventing you from working with kids directly? (e.g. medical, mental, legal etc condition)
6 Are there any other areas which you would like/able to contribute to the Birthday Programme or FFTH? (For example: Marketing, administration, fund-raising etc.)
